Patient and Wider Community Survey

Horfield Health Centre and Gloucester Road Medical Centre have recently joined up to form the Phoenix Primary Care Network, or PCN. This is part of the wider NHS programme to provide PCNs across the country. The idea is to work at a bigger scale, and to share good ideas between groups of GP practices.

The long term goal of PCNs is to improve the care we offer to our patients through better engagement with local community groups and the wider health and social care system.  You can find our more here: https://www.england.nhs.uk/primary-care/primary-care-networks/
